First of all, you have to click on this link https://setu.pmjay.gov.in/setu/checkpmjay.

In "Enter Your Mobile Number" enter your phone number which you have registered on this website and click on Get OTP button.

A 6 digit code will come on the mobile number you just entered, enter it and click on Sign In Button.

Then you get to see the Dashboard of PMJAY in front of you, from here you cannot apply for Ayushman Card, from here the option to apply Ayushman Card is not working, so click on the link given below

By visiting this Dashboard, you can apply for Ayushman card online by yourself, as well as check your name in the list, to log in here https://setu.pmjay.gov.in/setu/home You have to click on the /home link, then you will get to see the Dashboard in front of you, if you do not click Direct on this link, otherwise you will not be able to log in. Auto log in by following all the above steps.
