Fill this Registration Form with the name in whose name you are going to make Ayushman Card like State Name, District Name, Mobile Number, Email ID (any one can give it), Name Aadhar Card According, Gender, Date of Birth all After filling the details click on Submit Button.
As you click on Submit Button, you get to see some such message as User Has Been Created Successfully.

First of all, you have to click on this link https://setu.pmjay.gov.in/setu/checkpmjay.
In "Enter Your Mobile Number" enter your phone number which you have registered on this website and click on Get OTP button.
Then you get to see the Dashboard of PMJAY in front of you, from here you cannot apply for Ayushman Card, from here the option to apply Ayushman Card is not working, so click on the link given below.
By visiting this Dashboard, you can apply for Ayushman card online by yourself, as well as check your name in the list, to log in here https://setu.pmjay.gov.in/setu/home You have to click on the /home link, then you will get to see the Dashboard in front of you, if you do not click Direct on this link, otherwise you will not be able to log in. How to see your name online in PMJAY Ayushman Card List?
Follow the steps given below carefully to see your name in the list –
First check whether https://setu.pmjay.gov.in/setu/home is available on this URL or not, only then you can follow the steps given below.
Click on PMJAY CONSOLIDATED- DATA then click on Beneficiary Search by Vill/Town to check your name in PMJAY list.
As you click on Beneficiary Search by Vill/Town option and select State Name, District Name, Block Name, Village/Ward No, you will get the list below.
Check whether your name is there in the list or not, if you find your name in the list then you can apply for Ayushman Card.
How to apply for Ayushman card and PMJAY Card?
Follow the steps given below to get Ayushman card sitting at home online and apply for Ayushman immediately -
PMJAY CONSOLIDATED- DATA -> Search your name in the list of Beneficiary Search by Vill/Town for Ayushman Card.
Next to where your name is coming, click on Authenticate/Identify Button, from here you can apply for Ayushman card.
Then you will get to see Beneficiary Details, then you have to click on Next Button.
Enter any of your phone number in Mobile Number Filed, then select the option of OTP, then click on Get OTP Button.
Enter 6 Digit OTP which you would have received on registered mobile number and mail in Aadhaar card, then check box I Agree Button then click on Next Button.
Here you get to see the details of Aadhaar card, now you have to click on Next Button.
